I understand that in order for students to qualify for transportation services a bus will have to provide service in the area.

I understand that if assigned a stop location for elementary students the parent has to remain with the student until they have boarded the bus.

I understand that transportation is a courtesy service and can be taken away at any time due to route changes, ridership, safety concerns and discipline.

I agree that I will review the CSD Code of Conduct with my child.

I agree that the safety of my child while walking to and from school and waiting at the bus stop is my responsibility.

I agree that any change to my child's travel plans must be completed in writing and turned in at least 2 school days before the change is to occur.

I certify that I am the parent/legal guardian of the child listed on this form and that all of the information I have provided is true and accurate.
